Under Massachusetts state law, all campers must have a Health Form on file before attending camp. This includes a physical examination and immunization record signed by a doctor within 24 months of their camp attendance. Authorization forms are also required for the administration of prescription and over-the-counter medications. A Health and Permission Form can be found on our website under camp forms and will be mailed to you upon receipt of your registration form.
